ABSTRACT

Objective:To explore the correlation between different types of microcirculation alterations and the prognosis in patients with septic shock. Methods:This research employed a prospective observational study methodology for selecting subjects with septic shock. Side-stream dark field(SDF) was used to monitor the sublingual microcirculation to determine the total vascular density (TVD), perfused vessel density (PVD), the proportion of perfused vessels (PPV), and the microvascular flow index (MFI), heterogeneity index (HI) indicators. At the bedside, patients with microcirculation disorders were divided into four types: stasis, dilution, heterogeneity, and hyperdynamic. The 30-day survival status after enrollment and hemodynamics parameters were recorded. Results:A total of 64 patients with septic shock were selected in the study, including 18 cases of stasis type, 11 of dilution type, 18 of heterogeneous type, and 17 of hyperdynamic type. There were statistical differences in the mean arterial pressure (MAP) [stasis:(77±9) mmHg (1 mmHg=0.133 kPa), dilution:(80±11) mmHg, heterogeneity: (78±12) mmHg, hyperdynamic:(88±12) mmHg], TVD [ stasis:(10.84±3.01) mm/mm 2, dilution:(9.64±1.72) mm/mm 2, heterogeneity:(11.39±2.18) mm/mm 2, hyperdynamic: (11.87±2.67) mm/mm 2 ], PVD [stasis:(5.93±1.94) mm/mm 2, dilution:(6.86±1.48) mm/mm 2, heterogeneity: (8.31±1.78) mm/mm 2, hyperdynamic:(9.68±2.46) mm/mm 2], PPV [stasis:52.45 (46.25, 63.33)%, dilution:73.70 (61.50, 75.20)%, heterogeneity: 71.25 (67.95, 77.00)%, hyperdynamic:80.70 (77.25, 86.45)%], MFI(stasis:1.34±0.45, dilution: 1.70±0.38, heterogeneity:1.82±0.28, hyperdynamic:2.25±0.33), and HI [stasis:0.68 (0.51, 1.87), dilution: 0.57 (0.49, 0.64), heterogeneity:0.70 (0.59, 0.91), hyperdynamic: 0.40 (0.37, 0.52)] of the four types of microcirculation alterations. The cumulative survival rates in stasis, dilution, heterogeneity and hyperdynamic types at 30 day were 7/18, 4/11, 10/18 and 14/17, respectively, which in stasis and dilution types was significantly lower than that of hyperdynamic type (χ2=7.221, P=0.007;χ 2=6.764, P=0.009). Multivariate Cox regression analysis showed the type of microcirculation alterations (stasis: RR=4.551, 95 %CI 1.228-16.864, P=0.023; dilution: RR=4.086, 95 %CI 1.011-16.503, P=0.048), acute physiology and chronic health evaluation Ⅱ ( RR=1.077, 95 %CI 1.006-1.153, P=0.032) were independent prognostic risk factors. Conclusions:Microcirculation alterations are common in patients with septic shock, and it is hard to predict the types of microcirculation alterations with hemodynamics parameters. The prognosis of patients with septic shock is related to the types of microcirculation alterations, suggesting that routine monitoring of microcirculation might be helpful to guide hemodynamic therapy.

ABSTRACT

BACKGROUND: Safflower yel ow is known to treat diabetic nephropathy, can protect kidney function, reduce lesions, delay or prevent the development of diabetic nephropathy. OBJECTIVE: To further verify the effects of safflower yel ow on the kidney in rats with diabetic nephropathy and the impact of safflower yel ow on the expression of renal cel receptor of angiotensin II1. METHODS: Thirty rats were equally and randomly divided into three groups: normal control group, model group and experimental group. In the experimental group and model group, rat models of diabetic nephropathy were established. At 1 week after model establishment, rats were daily intraperitoneally given safflower yellow injection 27.8 mg/kg, once a day. In the model and control groups, rats were daily intraperitoneally given an equal volume of saline. All rats were administered for 23 weeks. The kidney-related biochemical indicators and pathological changes in kidney tissue and the expression of angiotensin II1 type receptor on kidney tissue of rats were observed. RESULTS AND CONCLUSION: (1) Hypertrophy index of experimental group and model group was significantly higher than that of control group (P < 0.05). (2) 24-hour proteinuria, glycosylated hemoglobin, total cholesterol, triglycerides, creatinine and urea nitrogen of model group were higher than that of control group (P < 0.05). 24-hour proteinuria, glycated hemoglobin, total cholesterol and urea nitrogen in experimental group were higher than that of control group (P < 0.05). 24-hour proteinuria, total cholesterol, triglycerides, creatinine and urea nitrogen in the experimental group were lower than that of model group (P < 0.05). (3) Glomerular hypertrophy, thickening of capillary basement membrane, cell proliferation, and mesangial widening were detected in the model group. Abnormal changes in the kidney structure were found in the control group. The extent of damage of kidney of histopathology in the experimental group was between the control group and the model group. (4) The expression level of angiotensin receptor II1 of experimental group and model group was higher than that of control group (P < 0.05). The expression level of angiotensin receptor II1 of experimental group was lower than that of model group (P < 0.05). These results indicated that safflower yellow has a protective effect on diabetic nephropathy; the mechanism of action may be associated with blocking kidney partial renin-angiotensin system.

ABSTRACT

BACKGROUND:Studies have found that β-casomorphin 7 can lessen oxidation of renal tubular epithelial cels and lower blood sugar in a rat model of diabetes. There is a few studies concerning the effect of β-casomorphin-7 on diabetic nephropathy. OBJECTIVE:To explore the effects of β-casomorphin-7 on renal injury in a rat model of diabetic nephropathy. METHODS:The 30 rats were equaly and randomly divided into control group, model group andβ-casomorphin-7 group. Twenty rat models of diabetes were established. In the model and β-casomorphin-7 groups, rats were intraperitonealy injected with streptozotocin (60 mg/kg). In the control group, rats were intraperitonealy injectedwith an equal volume of citric acid. Rats in the β-casomorphin-7 group were intragastricaly administeredβ-casomorphin-7 for 30 days. Rats in the model and control groups were given an equal volume of saline by intragastric administration. RESULTS AND CONCLUSION: (1) Compared with the control group, insulin levels decreased, glucagon, blood glucose, urine glucose, urine protein, serum creatinine, serum urea nitrogen levels, mRNA and protein expression levels of type I colagen and type IV colagen in kidney increased in the model group. (2) Compared with the model group, above indexes were apparently recovered in the β-casomorphin-7 group. These findings indicate that β-casomorphin-7 has a protective effect on renal injury in diabetic nephropathy rats.

ABSTRACT

PURPOSE: To assess the safety and efficacy of an ultramini nephrostomy tract, which we were using for the first time, combined with flexible ureterorenoscopy (URS) in the treatment of pediatric patients with multiple renal calculi. MATERIALS AND METHODS: Twenty pediatric patients (age, < or =6 years) underwent ultramini percutaneous nephrolithotomy (PCNL) combined with flexible URS. The group had multiple renal calculi, which were bilateral in 3 cases and were located in a total of 23 sites. The calculi were located in 2 calyces in 10 cases, scattered in more than 2 calyces in 7 cases, and limited to 1 calyx in 3 cases. The average patient age was 37.35 months (range, 14-68 months). The average stone diameter was 2.0 cm (range, 1-3.0 cm). In all patients, an ultramini nephrostomy tract was established under ultrasound guidance (dilated to F10) with simultaneous sheath placement. The flexible URS was placed into the collecting system during holmium laser lithotripsy. RESULTS: When ultramini PCNL was combined with flexible ureterorenoscopic holmium laser lithotripsy, the complete stone-free rate was 87% (20/23). The average level of hemoglobin decreased to 1.0 g/dL after the operation. No blood transfusions were needed. Levels of blood urea nitrogen, creatinine, and C-reactive protein were not significantly different before and after the operation. The average duration of hospitalization was approximately 4.85 days, and all cases were followed up for 6 to 12 months. No complications were found. CONCLUSIONS: Ultramini PCNL combined with flexible ureterorenoscopic holmium laser lithotripsy is a safe and effective treatment for children with multiple renal calculi.
